Zusammenfassung

Ohne auf die etablierten psychosomatischen Erklärungsansätze näher einzugehen, wird, anhand eines ausführlichen Fallbeispieles, in einer erfahrungsnahen Weise versucht, die entscheidende Schaltstelle des psychosomatischen Prozesses, d. h. den Ort, an dem es zur Weichenstellung zwischen psychisch sich äußerndem und psychosomatischem Symptom kommt, zu beschreiben. Mit erfahrungsnahe ist auch eine Darstellung gemeint, die die grundlegende Kluft zwischen der Außenperspektive, wie sie die Physiologie einnimmt, und der Innenperspektive des Erlebens zu überbrücken. Dabei wird die Entwicklung vom Affekt zum Gefühl als Ausgangspunkt für Störungen der Affektentwicklung bezeichnet. Kommt es zur Desintegration des Affekts, dann folgt daraus auch die Entkoppelung seiner Bestandteile, die Belastungssituation kann nicht mehr richtig interpretiert werden und gegen das Ungleichgewicht steuernde Handlungen werden verunmöglicht. Der Weg zum (psycho)somatischen Symptom ist gebahnt. Wenn die Medizin aus einem Missverständnis heraus versucht, eine Außenperspektive einzunehmen, so fixiert sie die erfolgte Bedeutungsentleerung, und kausale therapeutische Maßnahmen sind nicht mehr möglich.

Abstract

In this article the following basic question of psychosomatic medicine is investigated: in which control module of the organism is the decision made whether a psychic or a psychosomatic symptom is created. Describing this switch stand I tried to avoid the fundamental gap between an external (physiological) perspective and an internal (psychological) perspective by focusing the affect and its disturbances. By means of a detailed case report the decision making process between psychic and psychosomatic symptom genesis is described. Affects (synonymously: emotions) are (besides cognitions) of central importance for orientation. If an affect is disturbed, especially if it is desintegrated, the person is not able to identify the meaning of a strain and therefore not able to adapt it or to fight against it. Now the path to the development of a psychosomatic symptom is led.
